摘要:
目的 探讨星状神经节阻滞对三叉神经痛脑微血管减压术患者炎性反应的影响。方法 于2014年1月-2017年5月,随机选取本院收治的100例三叉神经痛患者进行前瞻性研究,按照计算机随机数字分组法将100例患者随机分为对照组与观察组,每组各50例,所有患者均接受微血管减压术治疗,观察组于右侧星状神经节部位注射罗哌卡因,对照组于相同部位注射生理盐水,再对2组患者进行全身麻醉; 比较2组患者不同时间的炎性反应、免疫功能指标水平、疼痛评分、睡眠质量指数,并比较两组患者的临床疗效。结果 观察组的总有效率显著高于对照组(P<0.05); 2组患者手术后的CRP、IL-6、TNF-α等炎性反应指标水平均较手术前显著降低(P<0.05),且手术后比较观察组的CRP、IL-6、TNF-α水平均低于对照组(P<0.05); 2组患者手术后的免疫功能指标水平均较手术前有所降低(P<0.05),但手术后观察组的免疫功能指标水平高于对照组(P<0.05); 2组手术后的疼痛评分、睡眠质量指数均较手术前显著降低(P<0.05),且手术后观察组的疼痛评分、睡眠质量指数均低于对照组(P<0.05)。结论 在三叉神经痛患者施行脑微血管减压术过程中对患者施行星状神经节阻滞可提高手术治疗效果,减轻患者疼痛感,改善其睡眠质量,还可调节免疫功能,抑制炎性反应。
Abstract:
ObjectiveTo study the effect of stellate ganglion block on inflammatory reaction of trigeminal neuralgia patients with cerebral microvasculardecompression.Methods A prospective study on 100 cases of patients with trigeminal neuralgia were randomly selected from our hospital From January 2014 to May 2017, in accordance with the digital computer random grouping method, 100 patients were randomly divided into control group and observation group, 50 cases in each group, all patients underwent microvascular decompression treatment, the observation group in the right stellate ganglion was injected with lidocaine, the control group in the right stellate ganglion was injected with the saline, then the patients of two groups were with general anesthesia. The inflammatory reaction parameters, immune function parameters, pain score, sleep quality parameters were compared at different times between two grouos, and the clinical curative effects of two groups were compared.Results The total effective rate of observation group was significantly higher than that of control group(P<0.05). The CRP, IL-6, TNF- alpha and other inflammatory response parameter levels of two groups were significantly lower after the operation than those before operation(P<0.05), and the CRP, IL-6, TNF- alpha levels were lower in observation group after the operation than those in control group(P<0.05). Immune function parameter levels of the two groups were significantly lower after the operation than those before operation(P<0.05), but after the operation the immune function parameter levels were higher in observation group than those in control group(P<0.05). The pain score, sleep quality parameter level of two groups were significantly lower after the operation than those before operation(P<0.05), and after the operation the pain score, sleep quality parameter level were lower in observation group than those in control group(P<0.05).Conclusion In the implementation of trigeminal neuralgia patients treated with microvascular decompression stellate ganglion block could improve the effect of surgical treatment, reduce the pain of patients, improve the quality of sleep, and also coudl regulate the immune function, inhibit the inflammatory reaction.
