摘要:
目的 探讨生酮饮食对KCNQ2基因变异导致的新生儿起病的发育和癫痫性脑病(KCNQ2-Developmental and epileptic encephalopathy,KCNQ2-DEE)的疗效及其早期应用的重要性。方法 回顾分析1例KCNQ2基因新发致病性错义变异(c.917C>T p.Ala306Val)导致的KCNQ2-DEE患儿的临床诊疗资料,并进行文献复习。结果 女性患儿,于出生后第2 d起病,表现为部分性发作及强直发作; 患儿经多种抗癫痫药物治疗发作未得到控制,出生1个月后予以生酮饮食治疗后癫痫发作得以控制,随访至第12个月4 d无癫痫发作但神经发育迟缓。以“KCNQ2”、“生酮饮食”为关键词共检索到6篇文献报道,生酮饮食均表现出明显的疗效,并且生酮饮食治疗开始的时间越早,患儿的语言及运动发育水平越接近神经发育里程碑。结论 对于KCNQ2基因变异导致的新生儿期起病的发育和癫痫性脑病患儿,药物治疗效果不佳时可早期采用生酮饮食治疗; 同时,需要更多病例研究或报道来进一步验证生酮饮食对KCNQ2导致的发育和癫痫性脑病的临床有效性。
