摘要:
目的 探讨自身免疫性脑炎(Autoimmune encephalitis,AE)患儿血清、脑脊液中核苷酸结合寡聚化结构域样受体蛋白3(Nucleotide-binding oligomerization domain-like receptor protein 3,NLRP3)表达水平与其临床特征、疾病预后的关系。方法 选取2019年1月-2023年9月本院收治的60例AE患儿纳入AE组,另选取同期60例病毒性脑炎患儿纳入病毒性脑炎组,检测并对比AE组与病毒性脑炎组血清和脑脊液中NLRP3水平差异; 分析AE患儿血清和脑脊液中NLRP3水平与其临床特征的关系; 随访6个月,根据疾病转归情况将AE患儿分为预后良好组与预后不良组,检测并对比预后良好组与预后不良组血清和脑脊液中NLRP3水平差异,绘制受试者工作特征(Receiver operating characteristic,ROC)曲线分析血清和脑脊液NLRP3表达水平应用于判断AE患儿预后的价值。结果 AE组血清和脑脊液NLRP3水平较病毒性脑炎组明显更高(P<0.05); 存在癫痫发作、运动障碍及睡眠障碍的AE患儿血清和脑脊液NLRP3水平较无癫痫发作、运动障碍及睡眠障碍的AE患儿明显更高(P<0.05); 与预后良好组比较,预后不良组血清及脑脊液NLRP3水平明显更高(P<0.05); 血清和脑脊液NLRP3水平单独及联合检测预测AE患儿预后的曲线下面积(Under the curve,AUC)(95%CI)分别为0.746(0.553~0.927)、0.696(0.442~0.939)及0.795(0.605~0.982),且联合水平检测对AE患儿预后的预测效能明显高于各指标水平单独检测(P<0.05)。结论 NLRP3水平在AE患儿血清和脑脊液中异常升高,与AE患儿癫痫发作、运动障碍及睡眠障碍等临床特征及不良预后密切相关,有望作为辅助评估AE患儿预后的预测指标。
Abstract:
ObjectiveTo investigate the relationship between the expression of Nucleotide-binding oligomerization domain-like receptor protein 3(NLRP3)in serum and cerebrospinal fluid of Autoimmune encephalitis(AE)children and their clinical characteristics and prognosis.Methods 60 AE children admitted to our hospital from January 2019 to September 2023 were selected as AE group, and 60 viral encephalitis children were selected as viral encephalitis group. The differences of NLRP3 levels in serum and cerebrospinal fluid between AE group and viral encephalitis group were detected and compared. The relationship between NLRP3 levels in serum and cerebrospinal fluid and clinical characteristics of AE children was analyzed. After 6 months of follow-up, AE children were divided into good prognosis group and poor prognosis group according to the prognosis of the disease. Then the differences of NLRP3 levels in serum and cerebrospinal fluid between good prognosis group and poor prognosis group were detected and compared, the value of serum and cerebrospinal fluid NLRP3 expression in judging the prognosis of AE children was analyzed by drawn Receiver operating characteristic(ROC)curve.Results The levels of NLRP3 in serum and cerebrospinal fluid in AE group were significantly higher than those in viral encephalitis group(P<0.05). The levels of NLRP3 in serum and cerebrospinal fluid of AE children with epileptic seizure, movement disorder and sleep disorder were significantly higher than those of AE children without epileptic seizure, movement disorder and sleep disorder(P<0.05). Compared with good prognosis group, the serum and cerebrospinal fluid NLRP3 levels in poor prognosis group were significantly higher(P<0.05). The AUC(0.95CI)of serum and cerebrospinal fluid NLRP3 levels alone and in combination for predicting the prognosis of children with AE were 0.746(0.553~0.927), 0.696(0.442~0.939)and 0.795(0.605~0.982)respectively, and the predictive efficacy of combined detection for the prognosis of AE children was significantly higher than that of each index alone(P<0.05).Conclusion The abnormal increase of NLRP3 in serum and cerebrospinal fluid of AE children is closely related to the clinical characteristics and poor prognosis of AE children, such as seizures, movement disorders and sleep disorders, which is expected to be used as an auxiliary predictor to evaluate the prognosis of AE children.
