摘要:
目的 探讨超低位去大骨瓣术联合亚低温在重型颅脑损伤患者中的疗效。方法 选择2015年8月-2018年8月本院收治的重型颅脑损伤患者153例,按照随机数字表法分为3组,各51例; A组采用常规去骨瓣术治疗,B组采用超低位去大骨瓣术治疗,C组采用超低位去大骨瓣术联合亚低温治疗; 对比3组术后患侧大脑中动脉PI指数、JSO2、Jlac水平变化及疗效。结果 C组术后第5、10 d的患侧大脑中动脉PI指数低于A、B组(P<0.05); C组术后第5、10 d的JSO2水平高于A、B组,Jlac水平低于A、B组(P<0.05); C组疗效优于A、B组(P<0.05)。结论 超低位去大骨瓣术联合亚低温治疗重型颅脑损伤患者可有效保护脑组织,减轻颅脑损伤,改善预后。
Abstract:
ObjectiveTo investigate the efficacy of ultra-low decompression and large cryoablation combined with mild hypothermia on patients with severe craniocerebral injury.Methods A total of 153 patients with severe craniocerebral injury admitted to our hospital from August 2015 to August 2018 were enrolled. According to the random number table, they were divided into 3 groups, 51 cases each. A Group was treated with conventional debridement. B Group was treated with ultra-low decompression and large bone flap. C Group was treated with ultra-low decompression and large cryoablation. The PI level of the middle cerebral artery, JSO2, Jlac levels and prognosis in the three groups were compared.Results In the C group the PI level of the middle cerebral artery and Jlac level were lower than those in the A and B groups on the 5th and 10th day after operation(P<0.05). In the C group the JSO2 levels were higher on the 5th and 10th day after the operation than those in A and B groups(P<0.05). The prognosis of C group was better than that of A and B groups(P<0.05).Conclusion Ultra-low decompression and large cryoablation combined with mild hypothermia in patients with severe craniocerebral injury could effectively protect brain tissue, reduce craniocerebral injury and improve prognosis.
