摘要:
目的 评估球囊导引导管辅助取栓技术治疗老年急性缺血性脑卒中患者的临床结局及作用。 方法 回顾分析取栓治疗的前循环大血管闭塞的老年患者(≥75岁)67例,分为非球囊导引导管(Balloon guided catheter,GBC)组和球囊导引导管组,比较2组的危险因素、闭塞血管、栓子逃逸、首通效应、取栓次数、血管再通时间、补救措施、血管再通程度、美国国立卫生研究院卒中量表(National institute of health stroke scale,NIHSS)评分、改良Rankin评分量表(Modified Rankin scale,mRS)评分、末次造影血流再通分级(Thrombolysis in myocardial infarction,mTICI)和并发症,采用Logistic回归分析影响临床良好预后的因素。 结果 非BGC组患者出现栓子逃逸者有12例(32.43%),高于BGC组的3例(10.00%)(P=0.03);非BGC组实现首通效应6例(16.22%),达到mTICI≥2b级20例(54.05%),90 d良好预后13例(35.14%),均低于BGC组的11例(36.67%),24例(80.00%),19例(63.33%)(P<0.05)。Logistic回归分析显示,治疗前NIHSS\[(比值比(Odds ratio,OR)=0.809,95%可信区间(Confidence Interval,CI)=0.717~0.912\]、取栓次数(OR=0.565,95%CI=0.342~0.946)、穿刺至再通时间(OR=0.985,95%CI=0.972~0.998)、使用BGC取栓治疗(OR=3.189,95%CI=1.169~8.698)与老年患者良好预后相关。 结论 对于老年急性缺血性脑卒中患者,球囊导引导管辅助取栓治疗可能是安全有效的治疗方法。
