摘要:
目的 探讨长链非编码RNA(Long noncoding RNA,lncRNA)核富集丰富转录本1(Nuclear enriched abundant transcript,NEAT1)在脑梗死患者血清中的表达水平变化,并分析其与血小板功能的关系。方法 选取2019年12月-2020年12月本院就诊并确诊为脑梗死患者85例为脑梗死组,同时选取体检健康者70例为对照组; 采用连续动态血小板功能检测法进行血小板粘附实验(Platelet adhesiveness test,Pad T)、血小板凝聚实验(Platelet aggre-gation test,Pag T); 采用电阻抗法测定血小板分布宽度(Platelet distribution width,PDW)、血小板压积(Platelet hematocrit,PCT)、血小板计数(Platelet count,PLT)、平均血小板体积(Mean platelet volume,MPV); 采用实时荧光定量聚合酶链式反应(Quantitative Real-time PCR,qRT-PCR)法对血清中lncRNA NEAT1表达水平进行检测; Pearson法分析lncRNA NEAT1表达水平与上述血小板功能指标的相关性; 受试者工作特征(Receiver operating characteristic,ROC)曲线评价血清lncRNA NEAT1水平对脑梗死发生的预测价值; 采用多因素Logistic回归分析法分析发生脑梗死的影响因素。结果 与对照组比较,脑梗死组患者血清中lncRNA NEAT1水平明显升高(P<0.05); 脑梗死组患者PDW,MPV,Pad T,Pag T水平均显著高于对照组,PLT,PCT水平低于对照组(P<0.05); 血清lncRNA NEAT1水平与PLT,PCT水平均呈负相关(r≤-0.305,P<0.05),与PDW,MPV,Pad T,Pag T水平均呈正相关(r≥0.277,P<0.05); ROC曲线分析显示,血清lncRNA NEAT1水平预测脑梗死患者的曲线下面积(Area under the curve,AUC)为0.956(95%CI=0.910~0.982),对应的敏感度为96.25%,特异度为92.00%; 多因素Logistic回归分析显示血清lncRNA NEAT1水平、PDW,MPV,Pad T,Pag T水平升高、PLT,PCT水平降低均是影响脑梗死发生的独立危险因素。结论 脑梗死患者血清中lncRNA NEAT1表达水平升高,且与血小板功能密切相关,可用于预测脑梗死的发生。
Abstract:
ObjectiveTo investigate the expression of lncRNA NEAT1 in the serum of patients with cerebral infarction, and to analyze its relationship with platelet function.Methods From December 2019 to December 2020, 85 patients who came to our hospital for treatment and were diagnosed with cerebral infarction were collected as the cerebral infarction group, while 70 healthy subjects were selected as the control group. Platelet adhesion test(Pad T)and platelet aggregation test(Pag T)were performed by continuous dynamic platelet function test. Platelet distribution width(PDW), platelet hematocrit(PCT), platelet count(PLT)and mean platelet volume(MPV)were determined by resistance resistance method; real-time fluorescence quantitative PCR was performed to measure the expression level of lncRNA NEAT1 in serum, the Pearson method was performed to analyze the correlation between the expression level of lncRNA NEAT1 and the above platelet function indicators; receiver operating characteristic(ROC)curve was performed to evaluate the diagnostic value of serum lncRNA NEAT1 level for cerebral infarction; multivariate Logistic regression analysis was performed to analyze the influencing factors of cerebral infarction.Results The serum lncRNA NEAT1 level in the cerebral infarction group was significantly increased than control group(P<0.05); the PDW, MPV, Pad T, and Pag T in the cerebral infarction group were significantly higher than control group, while PLT and PCT were lower(P<0.05). Serum lncRNA NEAT1 level was inversely correlated with PLT and PCT, and directly correlated with PDW, MPV, Pad T, and Pag T(P<0.05); the ROC curve analysis results showed that the area under the curve(AUC)of serum lncRNA NEAT1 level in predicting patients with cerebral infarction was 0.956(95%CI=0.910-0.982), the corresponding sensitivity was 96.25%, and the specificity was 92.00%; multivariate Logistic regression analysis showed that serum lncRNA NEAT1 level, increases of PDW, MPV, Pad T, Pag T, decreases of PLT, PCT were independent risk factors for cerebral infarction.Conclusion The expression level of lncRNA NEAT1 in serum of patients with cerebral infarction is increased, which is tightly related to platelet function and can be applied to predict the occurrence of cerebral infarction.
