摘要:
目的 探讨老年急性脑梗死患者血清趋化因子CC基序配体2(CCL2)、趋化因子CC基序配体3(CCL3)的表达水平与脑梗死体积、神经功能缺损程度的关系。方法 选取2016年8月-2017年8月于本院收治的老年急性脑梗死患者124例,并选择同期体检健康者78例作为对照组,应用Pullicino公式计算脑梗死体积将患者分为小梗死组(n=56)、中梗死组(n=41)、大梗死组(n=27),根据美国国立卫生研究院卒中量表(NIHSS)评分将患者分为轻度组(n=59)、中度组(n=37)、重度组(n=28),采取酶联免疫吸附法检测血清CCL2、CCL3水平,血清CCL2、CCL3水平与脑梗死体积、神经功能缺损程度间关系采用Pearson相关分析,影响急性脑梗死发生的危险因素应用多因素Logistic回归分析。结果 小、中、大梗死组血清CCL2、CCL3水平显著高于对照组,中、大梗死组血清CCL2、CCL3水平显著高于小梗死组,且大梗死组血清CCL2、CCL3水平显著高于中梗死组(P<0.05); 轻、中、重度组血清CCL2、CCL3水平显著高于对照组,中、重度组血清CCL2、CCL3水平显著高于轻度组,且重度组血清CCL2、CCL3水平显著高于中度组(P<0.05); 患者血清CCL2、CCL3水平与脑梗死体积分别呈显著正相关(r=0.349/0.481,P<0.05); 患者血清CCL2、CCL3水平与NIHSS评分分别呈显著正相关(r=0.493/0.650,P<0.05); 低密度脂蛋白胆固醇(LDL-C)、空腹血糖、CCL2、CCL3水平是影响急性脑梗死发生的危险因素(OR=1.912,1.624,2.272,2.082,95%CI=1.341~2.728,1.436~1.836,1.372~3.764,1.623~2.671,P<0.05)。结论 老年急性脑梗死患者血清CCL2、CCL3水平与脑梗死体积、神经功能缺损程度均呈显著正相关,可作为反映急性脑梗死患者的病情进展及严重程度的有效指标。
Abstract:
ObjectiveTo investigate the relationships between the expression levels of chemokine(C-C motif)ligand 2(CCL2),chemokine(C-C motif)ligand 3(CCL3)and cerebral infarct volume, the degree of neurological deficit in elderly patients with acute cerebral infarction.Methods 124 elderly patients with acute cerebral infarction were selected from August 2016 to August 2017, and 78 healthy people were selected as control group. Pullicino formula was used to calculate cerebral infarct volume, and the patients were divided into small infarction group(n=56), middle infarction group(n=41)and large infarction group(n=27), the patients were divided into mild group(n=59), moderate group(n=37)and severe group(n=28)according to the national institutes of health stroke scale(NIHSS), enzyme-linked immunosorbent assay was used to detect serum CCL2 and CCL3 levels, the correlations between serum CCL2, CCL3 levels and cerebral infarct volume, the degree of neurological deficit were analyzed by Pearson correlation analysis, multivariate Logistic regression analysis was used to determine the risk factors of acute cerebral infarction.Results The serum CCL2 and CCL3 levels in small, middle and large infarction groups were significantly higher than those in the control group, the serum CCL2 and CCL3 levels in the middle and large infarction groups were significantly higher than those in the small infarction group, the serum CCL2 and CCL3 levels in the large infarction group were significantly higher than those in the middle infarction group(P<0.05).The serum CCL2 and CCL3 levels in the mild, moderate and severe groups were significantly higher than those in the control group, serum CCL2 and CCL3 levels in moderate and severe groups were significantly higher than those in mild group, the serum CCL2 and CCL3 levels in severe group were significantly higher than those in moderate group(P<0.05). There were significantly positive correlations between serum CCL2, CCL3 levels and cerebral infarct volume(r=0.349/0.481,P<0.05). There were significantly positive correlations between serum CCL2, CCL3 levels and NIHSS score(r=0.493/0.650,P<0.05). Low density lipoprotein cholesterol(LDL-C), fasting blood glucose, CCL2 and CCL3 were the risk factors of acute cerebral infarction(OR=1.912,1.624,2.272,2.082, 95% CI=1.341~2.728,1.436~1.836,1.372~3.764,1.623~2.671,P<0.05).Conclusion Serum CCL2 and CCL3 levels in elderly patients with acute cerebral infarction were positively correlated with cerebral infarct volume and the degree of neurological deficit, which could be used as an effective index to reflect the progress and severity of acute cerebral infarction.
